The Type of Personal Information We Collect

We currently collect and process the following information:

Personal identifiers, contacts, and characteristics (for example, name and contact details).

Information required for dyslexia diagnostic assessment reports e.g., past diagnosis, test scores, medical conditions or medication that may affect testing etc.

 

How We Get the Personal Information and Why

Most of the personal information we process is provided to us directly by you for a diagnostic assessment.

We may if applicable receive personal information from an education establishment e.g., the school SENCo, but only with your prior knowledge, signed consent and according to all appropriate regulations.

We use the information provided to collate the evidence necessary to substantiate diagnostic judgements and produce dyslexia assessment reports.

We may share this information with the relevant education establishment e.g., the school SENCo, but only with your prior knowledge, signed consent and according to all appropriate regulations.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are:

(a) Your consent. You are able to remove your consent at any time. You can do this by contacting support@neurodiversityachievement.co.uk

(b) We have a contractual obligation.

(c) We have a vital interest.

 

How We Store Your Personal Information

Your information is securely stored for as long as is necessary for the relevant activity, or for as long as is set out in any relevant contract you hold with us.

We will then dispose of your information by securely deleting it from all systems.
