Assessment Terms and Conditions
Booking
Neurodiversity Achievement will liaise between the assessor and the client for the purposes of booking the assessment.
Neurodiversity Achievement reserves the right to change the assessor allocated if the assessor should become unavailable for any reason.
In order to book the assessment, your personal information and the information contained within your Pre- Assessment Questionnaire (PAQ) will be passed onto the assessor in order for them to carry out the assessment.
Privacy
Neurodiversity Achievement is committed to protecting the privacy of individuals. Accordingly, all personal data collected will be subject to our Privacy Policy.
Your assessment report will be held securely by Neurodiversity Achievement for 7 years (within your client file). We will then dispose of your information by securely deleting it from all systems. During this time, you will be able to ask for an electronic copy of the report. We would strongly recommend that you keep a copy of your report securely. After this time Neurodiversity Achievement will not be able to supply you with a copy of your report.
Should you require a copy of your assessment report, your report to be permanently deleted; or for changes/corrections made to it before this time; or to raise any concerns, please notify us at: reports@neurodiversityachievement.co.uk
Payment
Payment must be received before a booking can be fully processed and confirmed.
Cancellation policy
All cancellations must be notified via email with at least 48 hours’ notice of the assessment time.
If the client does not notify Neurodiversity Achievement of a cancellation with at least 48 hours’ notice, the client will be charged 50% of the fee to cover the costs incurred preparing the assessment.
If Neurodiversity Achievement are required to reschedule a booked assessment (e.g. if the assessor is sick), they will endeavour to give the client a minimum of 48 hours’ notice*
*Please note that it is the client’s responsibility to provide Neurodiversity Achievement with an emergency contact number at which they will normally be available.
